Introduction: Ventriculoperitoneal shunt (VPS) is most commonly performed surgical treatment for hydrocephalus by draining excessive cerebrospinal fluid (CSF) in ventricles to peritoneal cavity. Despite significant improvement procedure and being a relatively simple procedure, complications remain common. Aim: The aim of this paper report case perforated bowel presented with silence abdomen following VPS insertion. Case study: We protrusion distal catheter through anus silent abdomen, managed successfully minimal intervention. Patient, 11-months-old male infant, diagnosed congenital communicating placement was done at 6 months life, tip protruding from after 4 Patient scheduled removal VPS, where disconnected superficial abdominal incision, portion removed without resistance. Postoperative patient recovers well complication. Results discussion: Perforation rare only accounts 0.1%–0.7% exact pathogenesis spontaneous perforation unclear. Management secondary highly individualized, mainly depending on its clinical symptoms signs. Conclusions: Although insertion rare, it carries high mortality up 15% especially if unrecognized delayed treatment. Early detection appropriate management are key reducing related morbidity mortality.
